Name, date of birth, health/accessibility needs, contact numbers, address and email address(es) and location data.
Your personal information is processed to refer you to Heads-Up.
RBWM Council employees including Adult Social Services Front Door Team.
Your personal information may be shared with relevant Voluntary Community Groups and Heads-Up. The privacy notice for Heads Up can be found on the Heads Up website.
We store your personal information for up to two years.
