I am writing to make a request for recorded information under section 1 of the Freedom of Information Act 2000 concerning highway maintenance issues [...] enquiry reference TRE002064.

I request the following recorded information:

1. Original enquiry and complaint handling

- Records showing when enquiry TRE002064 was received, allocated, reassigned or escalated.
- Internal correspondence concerning the discovery that the enquiry had been directed to a former employee.
- Records of any internal investigation, review or corrective action following the council’s acknowledgement of this administrative failure.

2. Inspections and assessments

- Copies of inspection reports, site assessments and photographs relating to the reported highway maintenance issues.
- Records showing the dates on which inspections were undertaken and the findings recorded.
- Any recorded risk assessments concerning highway safety, visibility, vegetation and pedestrian access.

3. Contractor quotations and proposed works

- Records of requests for quotations issued to contractors.
- Copies of quotations received, including their dates and details of the proposed works.
- Any work orders, instructions, approvals or scheduled maintenance dates.
- Records of any cancellations, postponements or changes to the proposed works.

4. Internal correspondence and decision-making

- Internal emails, meeting notes and other recorded communications concerning the outstanding maintenance issues and associated delays.
- Records identifying the council departments or roles responsible for progressing the works.
- Any recorded decisions not to proceed with, or to postpone, the necessary maintenance.

5. Subsequent complaints and escalation

- Records of actions taken following the council’s decision to uphold my original complaint.
- Internal records concerning subsequent complaints or correspondence about the continuing lack of remedial action.
- Any recorded escalation to senior officers or management.
