For each of the below, please provide the contact for whoever holds commissioning and/or brokerage responsibility (i.e. someone able to discuss placement capacity and funding routes with external providers), rather than clinical or social work case-management staff, where these are different roles.
1. The name, job title, and direct contact details (email and telephone) of the lead commissioner responsible for adult learning disability and autism placements, covering both supported living and residential care settings.
2. The name, job title, and direct contact details (email and telephone) of the lead contact within your Adult Social Care Brokerage Team (or equivalent function responsible for sourcing and agreeing supported living and residential placements) for adults with learning disabilities and/or autism, along with the team's general email address and telephone number.
3. Please confirm whether your authority operates a framework or dynamic purchasing system (DPS) for Supported Living services for adults with learning disabilities and/or autism. If so, please provide the name of the framework/DPS and a link to the relevant provider portal or details of how to apply to join it.
4. Please confirm whether your authority operates a separate framework or dynamic purchasing system (DPS) for Residential Care services for adults with learning disabilities and/or autism. If so, please provide the name of the framework/DPS and a link to the relevant provider portal or details of how to apply to join it.
Where a named individual's direct contact details cannot be provided for either the commissioning or brokerage function, please provide the relevant team's email address and telephone number instead, along with the job title of those who monitor it. Please provide both the commissioning and brokerage contact details requested above, even where they sit with the same individual or team.
