Under the Freedom of Information Act 2000, please provide the following information relating to the premises at 19–21 Goswell Hill / Bridgewater Way, Windsor SL4 1RH (trading in the relevant period as Mantra Nightclub), for the period February 2005 to July 2012:
1.    The name of each premises licence holder for those premises during that period, with the dates each licence was granted, transferred, varied, or lapsed. (I understand a licence held by Big Red Monkey Ltd lapsed under s.27 Licensing Act 2003 in 2012, and a new licence was granted to Runcan Ltd on 13 June 2012.)
2.    The name of each person specified as Designated Premises Supervisor for those premises during that period, with the date of each DPS designation and each change of DPS.
3.    Whether a personal licence was issued by the Royal Borough to Michael David Garner (date of birth November 1987) at any time between 2005 and 2012, and if so its date of issue.
4.    Copies of, or the dates and applicant names for, any transfer or variation applications made in respect of those premises between 2005 and 2012.
