I am writing to formally request disclosure of environmental information held by the Council regarding the property at [House name], [Road name], Maidenhead, SL6 6[..].

Specifically, I request copies of all records held by the Environmental Health / Residential Services Department relating to inspections for damp and mould at this address between 11th Dec 2023 and 4th Aug 2025

This request includes, but is not limited to:

1. The full Housing Health and Safety Rating System (HHSRS) inspection report.

2. Copies of any Hazard Awareness Notices, Improvement Notices or Emergency Re-medial Action Notices served on the landlord/owner.

3. Officer site visit notes, time-stamped photographs, and internal correspondence re-garding the hazard assessment.
