We would like to request information relating to licenced animal operators in the area for which you are responsible. For each operator with a licenCe issued under the Animal Welfare (LIAIA) (England) Regulations 2018 would you please provide:

•       Licence number
•       Operator name
•       Type of Licence (Commercial boarding cats, Commercial boarding dogs, Home boarding dogs, Dog day care/Arranger, Dog breeding, Hiring of Horses, Exhibition animals, Selling as pets)
•       Star rating
•       Licence expiry date
•       Number and type of animals

•       Address Line 1
•       Address Line 2
•       Address Line 3
•       Postcode, Part of Postcode, Ward or town name

If possible, please provide this information in electronic format as an excel spreadsheet.
