1.    Provide evidence that the “Executive Director for adult services and Heath” or the Director of Public Health for RBWM ( as specified in 8.7 of  Guidance for 2003 Licensing Action section 182) was consulted on the premises license application received 16th August 2023 and issued on 12th September 2023  as PL107204 for the Odney Club Cookham.

2.    Please provide copies of any response, and its date,  from the request for consultation from the person  or office of as specified in 1.
