I wish to make the following request:

1. How many safeguarding concerns were raised by or about nursing or care home providers during the calendar year 2024?
Please include all concerns raised under the Care Act 2014, whether or not they progressed to a Section 42 enquiry. If only partial-year data is available due to recording system changes, please provide the exact time period covered.
2. Of those concerns, how many related to:
a) Serious injury
b) Unexpected death
c) Medication errors or administration issues
If exact figures are not recorded, please state whether the type of harm is routinely logged or categorised in safeguarding records.
3. How many of these concerns progressed to a Section 42 enquiry?
4. Of the Section 42 enquiries completed in 2024 relating to nursing or care homes, how many had an outcome recorded as ‘abuse or neglect substantiated’?
Please include the number substantiated under each abuse type if available (e.g., neglect, organisational abuse, etc.).
5. How many concerns or enquiries involved medication-related issues (e.g. errors, missed doses, incorrect prescriptions), and how many of these were substantiated?
If the council does not categorise incidents as 'medical negligence', please confirm whether medication-related incidents are coded or recorded separately and, if so, provide figures.
