I am writing to you under the Freedom of Information Act 2000 to request the following in-formation from the council. Please may you provide me with:

1)    The overall funding for this year allocated by the council to the NHS Health Checks programme 
2)    Is there a limit to the number of NHS Health Checks that GP practices can pro-vide and be reimbursed for by the council? 
3)    If so, what is this limit (per GP practice, per year)?
