I'd like to request the following, if possible:
1.    Does the County Council Reimburse NHS health checks in any service specifications with General Practices within your council?
2.    If YES to (2), what percentage of health checks were performed in 2023/24 financial year? 
Please may I have this as two columns: 
1) GP Practice Name + ODS code (Unique identifier)
2) Percentage of Health Checks performed in 2023/24
