Description: Case Managers assist people with ongoing and long term needs to live within the local community as independently as possible. A recovery focused approach is used.

Common conditions treated: services are for people with severe and / or enduring conditions: Obsessive Compulsive Disorder. OCD, Panic, Personality Disorders, Serious & disabling obsessions or Phobias, Post traumatic Stress Disorder (PTSD), Psychotic illness (such as schizophrenia), Serious Self harm, Substance Misuse (if combined with a severe mental health illness), Extreme and disabling anxiety, Bipolar Disorder (manic depression), Severe clinical depression.

You can access our services through: A GP referral to the Duty and Assessment Team at the Community Mental Health Team.
