Description: Provide intensive support to people with severe and enduring mental illness who may have a history of hospital admissions and multiple social problems who do not engage meaningfully and effectively with mental health services. The team supports users to maximise their confidence and independence focusing on the person's stability in the community, including finances, accommodation, daily living, medication and employment.

Common conditions treated: services are for people with severe and / or enduring conditions: bipolar disorder (manic depression); depression; personality disorders; schizophrenia; serious self harm.

You can access our services through: A GP referral to the Duty and Assessment team at the Community Mental Health Team.
