Community Team for People with Learning Disabilities
Who are we?
We are a team that consists of
- Nurses
- Care Managers
- Physiotherapists
- Occupational Therapists
- Speech and Language Therapists
- Psychologists.
The team is made up of employees from Health and Social Services.
What do we do?
We are here to Help, Advise and Support people with a learning disability and their carers.

Help to use health services i.e. Doctors, Dentists, attending hospital appointments etc.

Advise on how to take better care of health.

Support to learn new skills and make new choices

Help people with mental health and emotional difficulties

Advise on benefits and entitlements
-
Help people with physical and sensory disabilities

Help people who have difficulty communicating

Counselling for bereavement, loss, abuse

Help to find the most suitable accommodation i.e. residential, supported living or giving support when living at home with family

Support and help to carers
