Community Team for People with Learning Disabilities

Who are we?

We are a team that consists of

• Nurses
• Care Managers
• Physiotherapists
• Occupational Therapists
• Speech and Language Therapists
• Psychologists.

The team is made up of employees from Health and Social Services.

What do we do?

We are here to Help, Advise and Support people with a learning disability and their carers.

• Help to use health services i.e. Doctors, Dentists, attending hospital appointments etc.
• Advise on how to take better care of health.
• Support to learn new skills and make new choices
• Help people with mental health and emotional difficulties
• Advise on benefits and entitlements
• Help people with physical and sensory disabilities
• Help people who have difficulty communicating
• Counselling for bereavement, loss, abuse
• Help to find the most suitable accommodation i.e. residential, supported living or giving support when living at home with family
• Support and help to carers
