To notify change of address for a Personal Licence Holder please complete a Change of Address Notification Form (pdf 12 kb) and please ensure that you enclose a recent passport size photograph and the fee of £10.00. Cheques to be made payable to RBWM. Please return completed form to Licensing, RBWM, Town Hall, St Ives Road,Maidenhead, SL6 1RF.
