Processing is necessary for the purposes of preventive or occupational medicine, for the assessment of the working capacity of the employee, medical diagnosis, the provision of health or social care or treatment or the management of health or social care systems and services on the basis of domestic law or pursuant to contract with a health professional and subject to the conditions and safeguards.

Personal data is collected from you through a digital form.

What information is collected

The type of information collected includes; email addresses, personal experiences of using the health visiting and school nursing services. This may include references to health conditions.

How we use the information provided

We will use the information provided to assess our health visitor and school nursing services and understand resident's experiences of using the service. Information may also be used to inform future service provision. Anonymous statistics will be used to update elected members on the performance of the service.

The Public Health team.

Who we may share your information with

Your responses will not be shared. Anonymous statistics will form part of the council's reporting and analysis of the service. 

How long we store your information

Your email address will be deleted at the end of the consultation period and any personal information which has been input into the free text boxes. Statistics will be kept for five years.
