Retirement Forms
When you retire from the LGPS it is your employer's responsibility to advise us of your leaving details to enable us to calculate your pension benefits.
We will then provide you with a Retirement Pack containing the details of the your retirement options and enclosing the relevant forms for completion.

What's Contained in your Retirement Pack?
| Retirement Guide | A nearing retirement guide containing information on your retirement options. |
| Bank Details form |

| Lifetime Allowance form |
|
| Expression of Wish form | Nominate your next of kin to receive any death grant which may become payable. |
| Co-Habiting Partners Nomination form | Nominate your co-habiting partner to receive a dependant's pension in the event of your death. |
Please note we will also request a copy of your birth certificate and marriage or civil partnership certificates where applicable (photocopied documents are accepted).
