| The application must be completed by young people. In exceptional circumstances, an adult is able to complete the form on behalf of young people. |

CRITERIA |
|
| Maximum grant: £2,500 |
|
| 1. Applications must be completed by young people aged 11-19 (and up to age 25 for young people with learning disability). All parts of the form must be completed. |
|
| 2. Applications must have a CRB checked adult representative. The group also needs to have a bank account. |
|
| 3. Projects/groups/individuals that are funded must be located within the Royal Borough of Windsor and Maidenhead. |
|
| 4. All money received from the YVYC must be spent by 31 March 2013. |
|
| 5. Projects must provide feedback to the Youth Council on the success of their project by a specified date, which will be given once the bid is successful. |
|
| 6. All trips outside the UK and Ireland must benefit the community in which the trip is taking place or the community within RBWM. |
|
| 7. Groups can only have one successful application per year. |
|

SECTION 1: Group Details (Young People) |
|
| We need to know the group name or the individual applicant's name and age range. |
|
| Which category does your group come under? Applications will only be competing against other applications within the same category. A maximum of four applications from each category will be shortlisted and publicised in the Royal Borough of Windsor and Maidenhead's website for voting. Applications may be moved to a different category from which they have been submitted. Some examples of which category groups fall into are given below. |

| Schools: sports teams, arts groups, drama groups |
| Youth Groups: uniformed groups, religious youth groups, youth centre groups |
| Individuals: one person applying for funding for themselves |
| Targeted: children in care/leaving care; teenage parents; young carers; disabled young people; young travellers; young victims of domestic violence; young people living in social housing |

| We need to know how many young people have been involved in writing the application (only 11-19 year olds or up to age 25 for young people with learning disability) |

| You need to have a lead young person who is the contact person for the application. All emails and letters will be sent to this person. |

| You need to have an adult over 18 who will help and support you with your activity/project. The adult needs to have an up to date CRB (Criminal Records Bureau) check. For more information on the CRB visit www.crb.gov.uk |

| If successful, all details (except young persons' details) will be publicised online for voting purposes. |

SECTION THREE: Budget (what are you planning to spend the money on? |
|
| We need a breakdown of how the money will be spent and, where possible, we will need quotes. We also require a brief description of what you're spending the money on, eg, Venue - hire of Marlow Road Youth Centre - £75. You will also need to break down the costs if you are purchasing more than one item of equipment, going on more than one activity, or attending more than one training course. |

| If the total cost of what you want to do is going to be more than the amount you are asking for, you will need to tell us how much you already have and where you received your other money from. |
